Maternity UIF Document Checklist
- 13-digit bar-coded ID, smart ID card or valid passport
- UI-2.8 form for banking details
- UI-2.7 form for employer remuneration information
- UI-2.3 maternity application form or medical certificate, depending on the claim stage
- Medical certificate from a doctor or the baby’s birth certificate
- UI-4 follow-up form where required for continuation of payments
When to Apply
Official UIF maternity guidance says the application should be made before the birth or within 12 months after the birth of the child. The contributor must have been employed for at least 13 weeks before applying.
Maternity UIF is usually paid only if you receive less than your normal remuneration while on maternity leave.
